Broker Service – Community Liaison Officer 


Age Connects Cardiff and the Vale is looking to recruit the following post to work with the Cardiff Community Resource Teams. The role will include supporting older people living in the Cardiff localities.


The post is 14 hours a week, salary £8,099 * (funded until March 2021)

The post holder will need to work flexibly as part of an integrated team of health and social care professionals in a busy environment, acting as a conduit ensuring that information on Third Sector provision is readily accessible to relevant professionals and to broker service provision to meet identified need.


Knowledge of the range of third sector support across Cardiff & the Vale is essential along with an excellent understanding of the wider health and social care environment and experience of working in a multi-agency setting. Excellent communication skills are essential along with the ability to keep accurate computer records.
