The Home First Support Service provides personal care and reablement support to older people on a short-term basis as a means of either avoiding hospital admission or supporting discharge from hospital.
We are looking to recruit the following staff to the team.
HOME FIRST OFFICER/S (SUPPORT WORKER) * – x2 21 hours posts, £10,411 (funded until March 2019)
The Service operates between the hours of 8am & 6pm, 7 days week.
The Home First Support Serviceprovides personal care and reablement support to older people on a short-term basis as a means of either avoiding hospital admission or supporting discharge from hospital.
You will be a confident lone worker experienced in delivering personal care and promoting independence, flexible with good communication skills.
The post requires as a minimum a Diploma level 2 in Health & Social Care or equivalent or a commitment to achieve a qualification.
ICT skill are essential although additional training will be provided.
The Service is registered as a Domiciliary Care Agency with CIW.
