BROKER SERVICE - COMMUNITY LIAISON OFFICER- The post is 35 hours a week, salary £20,248 *

Age Connects Cardiff and the Vale is looking to recruit the following post to work with the Mental Health Services Team for Older People. 

In this exciting role you will be integrated within the Liaison Psychiatry Older People (LPOP) team in Llandough hospital. The role will include supporting older people living in the Cardiff & Vale localities.

The post holder will work in partnership with the LPOP team acting as a conduit to assist people over the age of 60 providing a seamless transition from hospital to community, ensuring that information on Third Sector is readily available to professionals and to pull together service provision to meet identified need.

Knowledge of the range of third sector support across Cardiff & the Vale is essential along with experience of supporting people with low level mental health needs. Excellent communication skills are essential along with the ability to keep accurate computer records.
