DSO services

The Age Connects Cardiff & the Vale Broker Service works proactively within the locality-based Cardiff Community Resource Teams (CRTs), C1 Vale Wellbeing Matters Service (C1V), and the Vale Community Resource Team.

The aim of the service is to maximise patient independence by providing rehabilitative support and information to older people and their families in their own homes:

  • On their discharge from hospital 
  • To prevent avoidable hospital admissions 
  • To help chose an appropriate care home, or support to address housing needs. 

The Service ‘sits’ within the integrated community teams in both Cardiff & the Vale and provides an effective interface raising awareness of Third Sector and Private Services ensuring information is readily available to all relevant professionals enabling support to be tailored to meet individual need with the aim of diverting demand from statutory services.

A one-stop shop person centred service which aims at keeping frail older people independent for as long as possible. The Community Liaison Officers will pull together a range of 3rd sector, private, or statutory services which will have the scope to provide the required support. This can be help needed with advocacy issues, benefit entitlement and other financial issues, loneliness and isolation, shopping, transport, condition specialist issues, housing, and provision of information on social activities and other topics.
