Cardiff & Vale Broker Service

Our Community Liaison Officers support older people, their families, and carers across Cardiff and the Vale of Glamorgan. We work alongside health and social care teams to help people access the right services, remain independent, and improve their wellbeing and quality of life.

We can help you access:

• Support following a hospital stay or during times of change
• Preventing avoidable hospital admissions
• Information and guidance on care and housing options
• Welfare benefits and financial support
• Energy advice
• Advocacy and specialist services
• Reducing loneliness and social isolation
• Shopping, transport, and community activities

By bringing together local voluntary, statutory, and private sector services, we help people make informed choices and access the support they need to live independently for longer.
